As a patient-centered healthcare provider, SouthCoast Health is passionate about providing care that is consistent with your values, priorities, and preferences. Should you be in a medical situation where you are unable to make or express your preferences, an advance directive or living will can give SouthCoast Health physicians the guidance they need to plan your care according to the preferences you have expressed.

If you already have an advance directive or living will, please share it and any updates with your SouthCoast Health physicians. If you haven't yet created an advance directive or living will, you can learn more about them through the resources below. We encourage you to consider creating one, because we want you to be a partner in your healthcare decisions, even in times when you can't speak for yourself.
