What follows is an important notice.
All SouthCoast Health locations will be CLOSED on Monday, September 7, 2025, in honor of Labor Day. SouthCoast Health offers an on-call physician service that is available any time our offices are closed. For this service, simply call your physician’s office phone number.

School Forms & Vaccine Records Policy

At SouthCoast Health Pediatrics, we are committed to providing exceptional care to your family, including timely access to necessary forms and records. To better manage these requests and ensure efficient service, we are updating our policy regarding school, camp, and sports forms, as well as vaccine record requests.

Effective June 1, 2025, the following guidelines will apply:

  • Forms Completed at the Time of a Well Visit – No Charge: Forms presented during a scheduled well-child visit will be completed as a courtesy, free of charge.
  • Forms Submitted Outside of a Visit – $30 Fee: Forms submitted outside of a visit (including those for school, camp, or sports) will be subject to a $30 fee per form. These will be completed and returned within 7 business days.
  • Rush Requests – $40 Fee: If you require a form to be completed in fewer than 3 business days, a $40 fee per form will apply.
  • Vaccine Records – Free at Well Visits / $10 Fee Otherwise: Vaccine records will be provided free of charge at well-child visits. If requested at any other time, a $10 fee per child will apply.
  • Plan Ahead to Avoid Fees: We encourage families to bring any required forms to their child’s well visit to avoid additional charges and ensure timely processing.
